How to become self-funded
Before becoming self funded you will need to:
- Fill out a State of Michigan application.
- Fill out an application for Excess Insurance.
To do the two items listed above a few different pieces of information will be needed:
Payroll history for the last 5 years. (Example)
- Payroll estimate for the up coming year. (Example)
- Loss Runs for the last 4-5 years
Eagle Claims Management will help fill out both the State and Excess application for you, and will then go over the other items that will be needed before approval from the State is given.
